2. This is (by definition) not a mineral. What are the main minerals that constitute it? Under what conditions does it form?
3. What are some uses for this "mineral"?
4. Where are most of the US's reserves of this "mineral" located? About how much of U.S. consumption is imported?

I'm so sorry to interrupt, but Bauxite (assuming that it was the mineral shown in the image) is listed under the category "Minerals" on the official list...? Is it a mineral or not? I have never done R&M before; I am sorry if I'm missing something.

Edit: Oops never mind, Bauxite is a group of a group of aluminum oxides.

Yeah, bauxite isn't a mineral, even though it is listed as one on the official list. Same thing goes for opal (it's amorphous->no crystal structure).

As far as the second questions for 2 and 4...

2. Most forms when a laterite soil in a wet (sub)tropical climate is leached of silica and other soluble components.
4. Over 99%
