What's the worst fail in scioly you ever had?

This regionals, I had to test the mousetrap (which I had never seen before the day of the competition) which ran backwards.... Twice..... First time we ran it backwards then my partner (who also was filling in) coiled it backwards when we ran it the right way

My partner and I somehow briefly forgot how our laser shoot template worked at state in Optics back in 2012, so the laser hit the edge of the mirror instead of the center and we failed the laser shoot section. We got 9th only through the test, having won the event at state the previous year and medaled in all the local tournaments that year, and worse the year we lost state by 5 points.

A few years ago at States the test for Dynamic Planet was a combination of a test off of the test exchange (which is a fail right there) and some extra basic information questions. I had done the test two nights before and there was even information on our cheat sheet that I added because of that test. Somehow we only got 11th. I still don't understand how we messed up that badly. My guess is we got something backwards that caused us to get a lot of the basic info questions wrong because they were all very similar.

I think I got last in forensics once. Note to self: don't do binder events with 3 hours of preparation.

That’s what happened to me too, except my partner thought he knew a lot (but he didn’t), and I was ploped into herpetology as a filler. I later found out we had NO HERPETOLOGY BINDER AT ALL.

Not realizing that all we had to do was a simple density lab at the Nebo Invitational...then not realizing that we could also get water.
