Should I add symptoms and take out type?

I would do Agent - Type - Incubation - Symptoms myself, and maybe throw in mode of transmission.

Well Agent and Transmission are both described in CAUSE
NAME....................................TYPE.................................CAUSE
EX: Anisakiasis........................ Parasite (Worm) ................Ingestion of uncooked fish
So after I finish this list im going to add Symptoms on the back I guess.

I suspect you could take an awful lot of tests before that ever showed up, while gastroenteritis would be in all of them. Do what you want, but personally I think that organizing by disease doesn't make a lot of sense.

What diseases are you guys putting in your charts? I have norovirus, e. coli, salmonella, and cholera. Do I need that parasites-from-fish one? And also, my format is Name, Cause, Incubation, Symptoms.

What would you suggest other than organization by Disease?

Im going with all the diseases on the CDC website, and then some equations and notes on the back

Ah, I see the source of confusion. Norovirus is not a disease, it is an agent. The disease it causes is gastroenteritis. Now that you mention it, though, I would probably have a mix of agents and diseases. I don't think you need anisakiasis, but you might want Paralytic Shellfish Poisoning and maybe Scombroid poisoning and ciguatera, and those are diseases rather than agents.

Also, E. coli (specifically strain O157:H7) causes hemorrhagic colitis.

on our cheat sheet we also included a chart from microbe mission that worked well at regionals. the whole thing was is size 4 font so we had room. if you have the space and a good chart you might want to include one for extra info and reference
