Tremolite, yes, fibrous tremolite is one of the recognized forms of asbestos, which is toxic and can cause lung cancer, among other diseases

Yup! Most of the tremolite that I find are the variations of hexagonite....

Correct, although it may just be slate - I'm not positive.
