Well... that first part is pretty self-explanatory. For structures you would have to know things like cross-sections of muscle fibers and parts of lungs, all that jazz.

Structure of organs for the respiratory system would be things like the pleural membranes, structure of an alveolus, etc. etc.

I would recommend making muscle flash cards. My test didn't have any ID questions on it, but I'm the exception to the rule. The tests are very in-depth across the board, I think.

Last year, when the systems were muscular and skeletal I encountered several questions on muscle tendons. Do you think tendon questions could appear this year as well?

Also, has anyone ever encountered an instance where he/she had to label deep muscles? It seems every test is on superficial muscles, but nothing in the rules eliminates deep muscles from the field of play.

I've never seen a tendon question, but it is entirely possible one could show up.
And you should only be tested on muscles on the list.

Amazingly they didn't ask about deep muscles.

I would say some tendon questions should be expected:
3.a.i The interaction of the skeletal and muscular systems (i.e., bones, joints, tendons and muscles) to allow movement.
What kinds of questions have you been getting on the effect of exercise on the respiratory system?
