Common Chokecherry, Prunus virginiana
1) X-disease, black knot
2) Tent caterpillars, I don't think there are any other major pests
3) a chokecherry is 1/4-3/8" in diameter that has shiny dark red or blackish skin with juicy, astringent or bitter pulp and a large stone
wufactor2012, Pawpaw leaves are untoothed ... so I don't think it's pawpaw.

pikachu4919 is correct :)

There is actually one more insect / pest but it's a minor one, often unnamed so I'll let it go ;)
